It seems sometimes notification emails are not being sent out.

This time, it was when someone posted news, and also when I added a comment to that news.
(Configuration (system and user level) is set to send emails on these events)

Anyone else see this occur?

I re-tested it using a smaller test project and it seems to work fine, but when another user creates a news entry in a more established and full project, it doesn't seem to always work.
