Attachments are updated but never shown in issue, images and office docs are being accepted

Strange issue, when I upload images or office docs as part of the issue I can see the attachment list.
If any other files (log, txt, xml. zip etc) are uploaded, the upload is successful and the redmine log shows no errors. However the attachments are never listed in the issue list.

I am running the latest 2.6.1 with MySQL and ruby 1.9.3 and httpd/passenger.

Additional data. What I noticed in the files older is that if I upload an image the file name is like timestamp_filename.png. But if it's a text Ike or some other file the file name is timestamp_<guid> and no extension.

Additionally, the attachments table is incomplete: for e.g. container_id, container_type and content_type are null. Also filename is funny as explained. I thought it might be to do with mime types, the mime-types gem is however installed.

See attached screenshot for the table entries.

Does that help to figure out what might be wrong?

Just in case someone else faces this problem. My site is behind cloudflare and some security settings there were stopping the upload. I fixed the security settings and its working sweet now.
