Thinking of upgrading a few versions.

Hi!
First time writer and first time on this forum, please be gentle. :)
I am thinking of upgrading from version 2.3 to the latest, but I first need to know if there are any changes in the database or similar which I need to be prepared for. Worried what might go haywire if I just do this huge leap, if my peers will be without system for a few days while I patch things or if the transition tends to be smooth.
As an extension to that question: If I opt to install the latest version on a clean system; would there be any tweaks in the database which could cause an issue?
I'm quite certain there are instructions for this somewhere, but I thought of posting here while searching.
