Hello,

I am facing an issue where sometimes when a new issue is created for example #945 a subsequent issue is then automatically generated(#946), this subsequent issue is identical in every way to the previous one. This does not occur every time but continues to be a problem, perhaps based on certain trackers that are being used.

Any idea what could cause this and how to fix it?
