Importing csv file is not successful

I tried importing an Issue file and finally get to see Subject cannot be blank, Tracker cannot be blank, Status cannot be blank for all the records in the Import Issue window.
I am using Redmine 4.0.4.
Have tried to assign some fields to the above items, but still get same result.
Somebody kindly guide me..
