there is a bug in redmine updat process.
if you click on update button at up-right corner, and do not change any thing, then click on update button, a new update on that issue will be logged. although nothing was updated.
it effects on making real report.

Why would that be a defect?
Why would you click on Update if you don't actually update any field value?

- Status changed from New to Closed
- Resolution set to Invalid
No update is logged with current trunk and a test was added for that time ago.
Maybe a plugin issue.
